If you have received or entered all the responses you require and you proceed with calculating your result now, your token will be spent, your survey participant data will no longer be editable and your new Basic Profile will be produced. If unsure, click Cancel. Otherwise, proceed.

Before you can produce your Basic Profile you need to:

1. Download and print off a copy of the participant survey for each of these people. Ensure you have printed all 4 pages (and that 4 pages are returned to you when complete).
2. Give the survey forms to each participant and encourage them to read the instructions at the top. If you have quite a number of paper survey participants, you may find it easier to gather them together to complete the survey, as it can be difficult to get surveys returned when they are sent home with people. As you want to encourage honest feedback, consider ways of protecting the privacy and confidentiality of the paper survey participants as is appropriate in your setting. You may choose to provide blank envelopes for the completed forms or perhaps have one person collect the forms and a different person enter the responses online. Speak to the participants about how you intend to protect their anonymity.
